I understand that Reformer Pilates classes involve physical activity that may place stress on the body, and I understand that participation in these activities carries an inherent risk of injury. I certify that I am physically fit, have not been advised otherwise by a qualified medical professional, and can participate in these classes.

I confirm that I have disclosed any and all relevant medical conditions, injuries, surgeries, or limitations that may affect my participation. I will inform the instructor before class if there are any new medical conditions or concerns, or if I experience any pain, discomfort or injury during the class.

I understand it is my responsibility to consult with a medical professional prior to engaging in any physical activities, and I have done so if necessary.

In the event of a medical emergency, I authorise the studio staff to seek medical treatment on my behalf if necessary. I agree to bear the cost of any medical treatment received.

I hereby release, waive, discharge, and hold harmless Cloud 9 Reformer Pilates Studio Limited, its owners, instructors, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action related to any injury, death, or damage that may occur during or as a result of my participation in Reformer Pilates activities.

I understand that Reformer Pilates involves the use of specialised equipment. I agree to follow the instructor's directions carefully and to use the equipment responsibly. I acknowledge that improper use of equipment may result in injury.

I understand and agree that Cloud 9 Reformer Pilates Studio is not responsible for any lost, stolen, or damaged personal property while participating in or visiting the studio.

I understand and agree to Cloud 9 Reformer Pilates Studio's 24 hour cancellation policy. Cancellations or rescheduling of classes must be made at least 24 hours prior to the scheduled class time. Failure to do so will result in the loss of the class credit, as determined by the studio's policy. No-shows and late cancellations are non-refundable and non-transferable.

This waiver constitutes the entire agreement between the parties and supersedes any prior agreements, representations, or understandings, written or oral, regarding the subject matter hereof.
